Point32Health to Insource Complex Care Management for Tufts Medicare Preferred Members; Ending Contract with Landmark 12/31/24

Point32Health is committed to delivering the highest quality of care, and we continuously evaluate the needs of our membership to improve health outcomes. After a thorough evaluation of the Landmark complex care program, we have made the strategic decision to bring this critical program offering in-house where we believe we can best meet the needs of our Tufts Medicare Preferred members. As a result of this decision, effective Jan. 1, 2025, the care of our most complex members will be insourced to a new complex care management program within Point32Health’s care management department.

 

The new insourced care model will provide enhanced services and support for members living with multiple, chronic health conditions. Initially, the program will focus on telephone support for members by an interdisciplinary care team. If members require in-home services, the care management team will collaborate with outside resources to ensure that in-home support is provided. 
 

In preparation for this shift, there is nothing required of members. Tufts Medicare Preferred members who are currently receiving care from a Landmark provider should experience no disruption in care. We’ll be seamlessly transitioning members who are actively engaged with Landmark to Point32Health’s care management team and will outreach to them based on the complexity of care needed. Additionally, our insourced complex care management team will identify members newly in need of complex care management services. Members will be able to reach the complex care management team during the transition via a dedicated phone line that will be shared with them by mail before the end of the year. 


There should not be any impact to the Tufts provider directories with this change. 


It is important to note that the Beth Israel Lahey Health Performance Network (BILHPN) contract with Landmark will remain in effect.
