Exciting plan benefits in 2025!

  • Our Access PPO plan includes:

    • $0 monthly premium
    • $1,500 of dental coverage with PPO Visa® Flex Advantage spending card - no network
    • [ENHANCED] Increased OTC benefit – a total of $520 a year to spend on OTC items
    • [ENHANCED] Reduced copay for outpatient diagnostic radiology services to $140 per day. (OON: 45% coinsurance) -ultrasounds remain at $100 per day


     

  • Our Smart Saver Rx HMO plan includes:
    • $0 monthly premium
    • $2,500 of dental coverage
    • [ENHANCED] Increased OTC benefit – a total of $560 a year to spend on OTC items
    • [ENHANCED] Reduced copay for outpatient diagnostic radiology services to $140 per day. (ultrasounds remain at $100 per day)

HRA - $100 reward
Submit enrollments, complete HRA’s, and receive a $100 fee. HRA fee will be paid out with the commission of the application. HRA now available through the Connecture Platform.

  • Platinum-100 applications and get $200 per app from 1st app sold
